Repeat Prescriptions

Please use your repeat prescription slip indicating the items you require; If you have lost the computer slip, clearly write your name, address and details of the prescription (copied from the bottle or tube).

You can request a repeat prescription by:  

• Using our online services
• Handing in your request at the reception desk,
• Leaving your request in the box outside the surgery or
• Sending your request to us by post. 

If you would like the prescription sent back to you please supply a stamped address envelope.

Always allow 48 hours notice (2 working days) to complete your request for a repeat prescription. Please note that if there is a query on your request this may take slightly longer.
